Family Advocacy Program (Psychiatric Rehabilitation – PRP)
An adjunct to outpatient treatment, the ESPS Family Advocacy Program (FAP) provides services that enhance the four domains of recovery:
Health – Family Advocates assist in learning about and working toward healthy lifestyles, managing chronic illnesses and accessing needed health care services
Home – Through home visits, Family Advocates focus on family strengths and challenges in assuring a safe and nurturing home environment
Purpose – Family Advocates assist individuals in achieving their educational and vocational goals by providing support, identifying and accessing resources for training and employment information
Community – By assisting in the identification and linkages with natural community supports, resources and opportunities, Family Advocates help individuals and families move toward greater participation in and contribution to their communities
FAP activities focus on skill building in the areas of:
Skill Building
Self management and daily living
Communication
Assertiveness
Problem solving
Social and interpersonal
Understanding their own health and behavioral health issues and managing their own care
