We are looking for an highly skilled (Male) Community Support Specialists (due to certain increased demand) to join our team here at Eastern Shore Psychological Services!
As a Community Support Specialist, he is responsible for implementing the rehabilitation activities outlined in the client’s Individual Rehabilitation Plan. Demonstrates flexible and efficient time management and ability to prioritize workload in accordance with the ESPS Mission Statement and guiding principles and that the maintains a spirit of ‘Mission Driven Excellence’ in all aspects of their position.
In collaboration with the individual and the PRP Supervisor / Lead Family Advocate, develops and implements a written individual rehabilitation plan that outlines desired outcomes and identifies strategies to achieve them.Facilitates through professional relationships and referrals, the individual’s utilization of available support systems and community resources to meet identified needs.
Participates in PRP services including the implementation and coordination of activities related to the rehabilitation plan; consults with the Therapist / Clinical Supervisor.
Provides individual and group activities that facilitate progress toward IRP goals including the development of social skills, anger management skills, and problem solving skills.
Maintains documentation in client records in compliance with all applicable state and federal regulations and ESPS clinical standards within required timeframes.
Adheres to ethical and behavioral standards of conduct and participates in continuing professional development seminars, workshops, or training sessions to increase knowledge and enhance job skills.
Ensures that client confidentiality and rights are maintained.
Responsible for maintaining assigned productivity standards.
Provide transportation to clients and make community and home visits as required by client IRPs.

Education: Minimum Requirements
A bachelor’s degree in a health-related field; or
An associate’s degree in a health-related field
30 hours, or their equivalent, of college credit toward a bachelor’s degree in a health-related field.
1 year of work experience in a supervised mental health setting
Licenses/Certifications: Current CPR, first aid and infection control certification, preferred
